Ethical approval was obtained from the Graduate School of Medicine’s Research Ethics Committee at The University of Tokyo (SN 11990). In Uganda, ethical approval was obtained through the Institutional Review Board of Makerere University School of Health Science (P578) and the Uganda National Council of Science and Technology (SS4656). Participation in this study was voluntary. All caretakers who accepted to participate provided written informed consent. Each interview was undertaken in a confidential and secure environment.
