: Approval for the overarching B-Healthy study was obtained from the Health Research Ethics Committee of the Faculty of Health Sciences at the North-West University (NWU-00049–15-A1), as well as for the specific outcomes of the PhD study (NWU-00049–15-A1-03) associated with the B-Healthy study and the provincial Department of Health of the North West Province. All data collection was performed according to the Helsinki Declaration (2013). All participants gave informed consent before participation. Native Tswana speakers served as gatekeepers and read the informed consent to the participants that could not read. Participants drew a cross on the informed consent form to indicate their consent for participation in the study, instead of a signature. All participants were free to withdraw from the study at any time. Partial confidentiality was ensured for participants during FGDs. A numbers system was used during FGDs, to ensure the anonymity of participants during data analyses.
