: Ethics approval was obtained by the Arizona State University (ASU) Research Integrity and Assurance Institutional Review Board (STUDY 00001155). The ASU institutional review board approved our consent and assent process. Prior to commencing data collection, all participants received information about the research, potential harms and benefits, and the right to withdraw at any time. Confidentiality was secured by having participants select a family and individual pseudonym for all research-related field notes and to sign the consent forms. The research team signed the consent forms with their real names. This research used subject identification numbers to track biological specimens and survey responses. All adult participants provided written informed consent. Parents signed a form that combined adult consent and permission for children to participate. Youth ages 6–17 provided written assent. Youth < 6 years of age provided verbal assent. All methods were according to the ethical principles of the Declaration of Helsinki and The Belmont Report, and approved by the ethical board above.
: By signing the consent forms, participants agreed to have their pseudonyms used in scientific reports and conference presentations. Their consent form also indicated that anthropometric, survey, and salivary data would be reported in aggregate. No information about their personal identifiers would be reported in scientific reports or conferences.
