: All the proposed study methods will be performed in accordance with the relevant guidelines and regulations and have been approved by the local ethics committee. Considering the study involves multiple phases using different research methodologies, ethics approval was sought for each phase separately. Phase 1 of the study did not require any approval as it involved analysis of secondary data and the University of Auckland Human Participant Ethics Committee has approved the Youth2000 surveys (022244, 2011/206, 2005/414, 1999/014). The Auckland Health Research Ethics Committee has approved Phase 2 (AH23606), Phase 3 (AH24474) and Phase 4 (AH25686). Written informed consent will be obtained from participants of study Phases 2 (stakeholder interviews), 3 and 4. All participants will be given an opportunity to ask questions of the research prior to taking part in it. Participants will be clearly informed that participation in the study is entirely voluntary, and they can withdraw from the study at any time without giving a reason.
