They also declare that all procedures contributing to this work have been performed in accordance with the ethical standards of the relevant national and institutional committees on human experimentation and with the Helsinki Declaration of 1975, as revised in 2008. Ethical approval was obtained from the Regional Committee for Medical and Health Research Ethics, South East Norway (REK 2014/1970) and all registry owners approved utilization of their data. The requirement for informed consent was waived by the Regional Committee for Medical and Health Research Ethics, South East Norway (REK 2014/1970) because of the nature of the study. The current study was based on routinely collected and anonymized register data. The law regulating official statistics in Norway implies that anonymized register data can be used for research. The law of statistics combined with the Ethics Committee of South East Norway thus confirmed that informed consent was not necessary.
