: Ethics approval was obtained by the University of Waterloo Research Ethics Committee before the study began for both the development and validation study (project # 42486). Methods for both the development and validation studies were carried out in accordance with relevant guidelines and regulations outlined in the Government of Canada Tri-Council Policy Statement for Ethical Conduct for Research Involving Humans. For the development study, researchers were provided with information that could identify individual participants during or after data collection. However, this information was only available to individuals listed on the ethics application and remains confidential on a password protected computer. All participants provided informed consent for the recording and use of quotes in publications. Pseudonyms were used to maintain anonymity. For the validation study, ethics approvals were given to collect demographic information for the purpose of analysis from Leger. Informed consent was obtained prior to respondents’ completion of the survey. No information was provided to the researchers in the validation study that could identify individual participants during or after data collection.
