Ethics approval and consent to participate

: Ethical approval for this study was received from the Human Research Ethics committee at the Children’s Health Queensland Hospital and Health Service (HREC/14/QRCH/136) and the Human Research Ethics committee at the University of Queensland (2014001176). After discussing the study, parents with children who were eligible and interested in participating provided written informed consent. Children aged between 7 and 13 years also provided written assent.
