: The COSYCONET study complies with the Declaration of Helsinki and Good Clinical Practice Guidelines and has been approved by the ethics committee of the medical faculty of the Philipps-Universität Marburg, the local ethics committees of the participating centers (a list of all participating study centers can be found here:ExternalRef removed) and by the concerned data security authority (data security agency of the federal states of Hesse, Baden-Württemberg, Lower-Saxony, and Saarland). This approval covered the subsequent data analyses as performed here. All cohort participants gave their written informed consent.
