: The study was approved by the Maseno University Ethics Review Committee (Maseno, Kenya; ref: MSU/DRPI/MUERC/00433/17), the Western Institutional Review Board-Copernicus Group (Seattle, USA; tracking number: 20171427), and the University of Alberta Research Ethics Board Biomedical Panel (Edmonton, Canada; ref: Pro00086635). Accompanying parents or legal guardians provided written informed consent and patients > 7 years of age provided assent at the time of enrolment.
