Ethics approval and consent to participate
: Ethical approval was obtained from the Dutch Medical Ethics Committee of Rotterdam (ref. number NL46385.101.15). Approval of local Medical Ethical Committees was obtained in all participating hospitals separately.All patients will be asked for written informed consent on standardized forms. The data will be kept de-identified on forms and in computers inaccessible to unauthorized persons. Data is securely accessible to members of the research team. In compliance with government regulations, personal data will be kept de-identified for a further period of 15 years after the end of the trial, after which they will be deleted.
