Ethics approval and consent to participate

: Other than filling out questionnaires, patients did not suffer any harm or inconvenience by participation. Patients gave written informed consent. This is a questionnaire study, and all patients receive treatment as usual. Consequently, approval from an ethics committee is not required, according to the Danish national guidelines [CitationRef removed]. The trial is registered with the Danish Data Protection Agency (Central Denmark Region, journal no. 1–16–02-23-17). The study follows Responsible Research and Innovation and the Danish Code of Conduct for Research Integrity. Reporting and storage of data are in accordance with EU Guidelines on FAIR Data Management in Horizon 2020 (Version 3.0, 26 July 2016).
