The study was approved by the research ethics board of the McGill University Health Center (project number 2014 − 1059). This study is a cohort study of provincial registry data. Data were anonymized for researchers. Additionally, researchers ensured that none of the data analyses could lead to readers being able to identify an individual patient. As such, no informed consent was needed and informed consent was impossible to obtain. The research ethics board of the McGill University Health Center granted exemption from requiring informed consent (project number 2014 − 1059).
