: The protocol underwent Māori (indigenous New Zealanders) consultation, which fostered input into this study, and scientific peer review internally by the Department of Women’s and Children’s Health, University of Otago, and externally by the Cure Kids Medical and Scientific Advisory Committee. The Southern Health and Disability Ethics Committee approved this study (17/STH/240). All district health boards approved recruitment and conduct of the study at their site.Written informed consent will be obtained from adult participants, a parent/guardian of adolescents aged 13–15 years will provide written informed consent, and adolescents aged 13–15 years will provide written assent.
