: Study A was approved by the local ethics committee at Lund University, and written, informed consent was obtained from all participants. Study B was approved by the Copenhagen and Frederiksberg Regional Ethics Committee (ref. no. 01–262546) and informed written consent was obtained from all of the subjects before participation. Both studies were conducted according to the principles of the Helsinki Declaration.
