Ethics approval and consent to participate
: The LASA study is conducted in line with the Declaration of Helsinki, and was approved by the Medical Ethical Committee of VU University Medical Center (reference numbers 92/138; 2002/142; 2012/361). In addition, this study was evaluated and approved by the LASA evaluation committee who declared that the study is in line with the overarching aims of LASA that have been communicated with its participants. All participants provided informed consent at baseline. From 2011/2012 onwards informed consent was provided before every new measurement.
