Ethics approval and consent to participate
: According to the Dutch Medical Research Involving Human Subjects Act, this study did not require approval by a medical research ethics committee in the Netherlands. Respondents were researchers and professionals, not patients and data collection methods consisted merely of interviews and questionnaires. We followed the ethical principles for medical research involving human subjects as laid down in the Declaration of Helsinki and adopted by the World Medical Association (WMA Declaration of Helsinki 2000). Each respondent was adequately informed of the aims and methods of the study and written consent for participation in this study was obtained from the respondents. No personal information has been used and the individual’s identity has been protected by removing any personal identifiers from the data. Codes were used to designate the respondents to guarantee their anonymity.
