The parent study was reviewed by the University of Wisconsin-Madison Health Sciences Minimal Risk Institutional Review Board (Protocol Number 2014-1326), and the Institutional Review Board determined that the parent study was qualified for exemption. An information page was provided to potential participants with key elements of informed consent, and participants were informed that completion of the survey implied consent to participate.
