Ethics approval and consent to participate
: Ethical approval was granted by the University of Ibadan/University College Hospital (UI/UCH) Ethics Committee (NHREC/05/01/2008a) and the study was assigned number UI/EC/14/009. Ethical review at the UCH was renewed for the years 2016, and 2017, respectively. Ethical approval was also obtained from the Oyo State Ministry of Health Ethics Committee, Ibadan, Nigeria. There was written permission to conduct the study at the University College Hospital, Obstetrics and Gynaecology unit, Adeoyo State Hospital, Yemetu, and Ibadan South-East Local Government. All participants gave their written informed consent.
