Ethics approval and consent to participate
: Ethical approval for these studies was obtained from the Institutional Review Board of The University of Texas at Austin [study numbers 2011-11-0025 and 2014-07-0059]. An information sheet describing the study purpose, procedure, risks, and benefits was offered to each survey respondent and a signed copy was obtained from them before completing the survey as part of the informed consent procedure. We destroyed names and contact information of potential qualitative interview participants when the study was completed. We did not collect signed consent forms from qualitative interview participants to avoid maintaining documentation of their names. To further minimize the risk of a breach of confidentiality, we also obtained a Certificate of Confidentiality from the National Institutes of Health. An information sheet describing the study purpose, procedure, risks, and benefits was offered to each interview participant, the interviewer explained these details, and verbal consent was obtained.
