Ethics approval and consent to participate
: The study was approved by the Higher Degree Research and Ethics Committee at Makerere University school of Public Health. Further permission to conduct the study was sought from the district health office of Kabarole district and from in charges of health facilities. We obtained written informed consent from all participating women. In order to document the informed consent process, participants would either sign or provide a thumbprint on the informed consent form approved by the Research and Ethics Committee. All signed or thumb printed consent forms were stored in locked file cabinet.
