Pasteur Teaching Hospital in Slovakia (EK 2020/EK/04024), Due to the anonymous nature of the survey and the ability of participants to elect to respond to the survey or not, the ethics committee did not request a separate consent form because completion and return of the questionnaire was considered an implied consent to participate.
