: The national institutions that conducted the surveys in each country were responsible for seeking ethical clearance on the questionnaires and protocols used. These were reviewed and approved by the Institutional Review Board of ICF International Ethical committee and relevant in-country ethical boards. So, ethical approval was not required for our study. Additionally, informed consent and anonymity do not apply to our study as these were assured before and during data collection by the respective implementing national bodies. The datasets were downloaded from the DHS website ExternalRef removed with permission for access and use sought from MEASURE DHS, conditional on using the datasets for this study. Any other requirement to use the data would have to be sought from the MEASURE DHS program team.
