: The study was conducted in adherence to the Declaration of Helsinki. According to Danish legislation ethical approval is not required for surveys and individual-level linkage of data from surveys and administrative registries is allowed without further consent when it is for research purposes and when thoroughly ensuring that results are presented in an anonymized way [CitationRef removed]. All surveys until 2013 were approved by the Danish Data Protection Agency and the most recent surveys were approved by SDU Research & Innovation Organization (SDU RIO). Since 2016, SDU RIO have examined and approved all scientific and statistical projects at the University of Southern Denmark according to the Danish Data Protection Regulation. In the letter of introduction for each survey wave the purpose and the content of the survey was described and it was emphasised that participation was voluntary. Furthermore, the letter emphasised that data solely will be used for research and statistical purposes. Individuals at least 16 years of age, can make an independent decision to participate in surveys without the consent of parents or another legal guardian according to Danish legalisation and ethical constraints. Completing the questionnaire was considered as consent for participation, hence informed consent was obtained from all participants. Moreover, according to Danish law, individual-level linkage of data from for example, surveys and administrative registries is allowed without further consent when it is for research purposes and when thoroughly ensuring that results are presented in an anonymized way.
