: Ethical Approval was gained from the Plymouth University Faculty of Health and Human Sciences Research Ethics Committee prior to commencement of the study (ref 15/16–525).As stated on the ethics protocol, consent to participate in the study was implied through completion of the survey. Participants were informed of their right not to participate, or to withdraw at any point, in the written information provided to them at the beginning of the survey.
