: Sierra Leone Ethics and Scientific Review Committee approved the study. The Norwegian Regional Committees for Medical and Health Research Ethics was consulted, but a formal application was not considered necessary by the committees because the study was regarded as a quality assurance project.A form declaring informed consent was signed by all interviewed participants.
