Ethics approval and consent to participate
: According to Danish regulations, the study did not need formal approval from a health research ethics committee. The study complies with the World Medical Association Declaration of Helsinki, including providing informed consent to study participation: All participants in the questionnaire study gave consent by active participation. All participants in the audiotaped structured discussions gave verbally informed consent to participation and publication of anonymised results. The informed consent was given verbally according to Danish regulations and was noted by the researchers. ExternalRef removed<b>.</b>The National Committee on Health Ethics in Denmark has been consulted and according to Danish law, this type of study needed no further approval from the committee. The research ethics committee at SDU was started in late 2018 and only provides approvals for projects initiated subsequently. Hence, we have strictly followed all regulations pertinent to our study.
