: The study was approved by the Regional Ethical Review Board in Stockholm (Dnr 2015/270–31/5).The participants were initially contacted by email and if they were willing to participate they obtained a document with information about the design and purpose of the study, including the following consent to participate statement: “Note that your participation is completely voluntary and that you, if you agree to participate, can withdraw your participation without presenting any reason for doing so. You also have a right to contact us at any time if you want to withdraw your participation. Participation in the study will not affect your employment or your professional position.
