: The study was conducted in accordance with the Declaration of Helsinki after approval of the Research Ethics Committee (REC) of the King Faisal Specialist Hospital and Research Center (KFSH&RC). All respondents provided verbal informed consent as approved by the REC. A verbal rather than a written informed consent was requested by the REC because the study does not present more than minimal risk of harm to respondents, does not involve any procedures for which written consent is normally required, and for confidentiality protection, as a consent document would be the only identifiable link between respondents and the study.
