: In accordance with the Helsinki declaration [CitationRef removed], the research project was conducted in respect of informed consent, respect for personal integrity, the right to privacy and respect for personal integrity and dignity. All participants gave written informed consent, after receiving both verbal and written information about the project, including their right to withdraw their consent at any time.The project is registered with the Danish Data Protection Agency (2011–16/12762) in accordance with Danish law. Danish research ethics committees do not review qualitative studies. This was confirmed in an email from The National Committee on Health Research Ethics in Denmark received by the first author on 3 December 2014.
