: This project has ethics approval from the Human Research Ethics Committee of the University of Sydney, HREC project number 2015/397.The quantitative survey of the general public has ethics approval from the Human Research Ethics Committee of Swinburne University, SHR Project 2017/180.Participants provided verbal consent to participate anonymously in the quantitative telephone survey, a procedure that was approved by the Swinburne University Human Research Ethics Committee and endorsed by the University of Sydney Human Research Ethics Committee.For the in-depth interviews, participants received an Information Sheet and Consent Form via email or post. They were offered the choice to return the signed Participant Consent Form, or to provide verbal consent at the start of the interview; this procedure was approved by the University of Sydney Human Research Ethics Committee.
