Ethics approval and consent to participate
: The study required written informed consent from participants and they were informed that participation was voluntary. There was minimal risk to participant lives as a result of the study except for loss of time. The interviewer may have asked some sensitive questions about reason for non-participation and income that made them uneasy. The principle investigator was readily available to respond to all patient concerns. Ethics approval was obtained from the Mulago Hospital Research Ethics Committee (MREC 991) and approved by the Uganda National Council for Science and Technology (HS SS 5013).
