: Ethical approval for the study was granted by the North West - Greater Manchester East NHS Research Ethics Committee (reference 15/NW/0792) and from the local NHS research and development board.Practices consented to take part in the study and began to receive the SMASH intervention thereafter. Additional written informed consent was taken from individual participants prior to each interview, which included consent for the interviews to be audio recorded and transcribed verbatim and for anonymised quotations to be used in reports.
