: All methods were carried out in accordance with relevant guidelines and regulations. The Medical Ethics Committee of the Erasmus MC (Rotterdam, The Netherlands), has reviewed the research proposal for the entire project (of which the study reported here is a part) and assessed that the rules laid down in the Medical Research Involving Human Subjects Act, do not apply (MEC-2018–151). Research governance approval was received from all participating hospitals. Patients provided written informed consent before their consultation was recorded.
