: Ethics approval to conduct this current study was not required as approval to use the results drawn from the surveys conducted in Australia, Canada and Scotland is covered in the original ethics applications: Survey data is kept “indefinitely for scholarly priority” in accordance with the AustralianNational Health & Medical Research Guidelines(2007). Access to the Swedish sample was provided by the Swedish Association of Health Professionals.
