: The survey was carried out in accordance with research governance regulations and guidelines in each country. In Australia, the study was approved by the “Human Research” Ethics Committee of the Australian Catholic University (2019-129E). In the UK approval was granted from the Health Research Authority and Health and Care Research Wales (HCRW) REC (19/HRA/1476), and the University of Central Lancashire “STEMH” Ethics Committee (STEMH 942). Study approval in the UK was also granted from the Health Research Authority and from individual hospital trusts. Participation was voluntary and informed consent was obtained from all subjects. Procedures were put in place to ensure the anonymity of hospitals completing the questionnaires, such as the use of codes to link the hospitals with the questionnaires, which were managed by the researchers involved in the study.
