Ethics approval and consent to participate
: The household surveys were approved by the Ethics Committee/Institutional Review Board of School of Public Health, Fudan University. During the household surveys, written informed consent forms were sent to the respondents to inform them on the research purpose and survey contents, and to obtain their consent to participate in the survey. The health facility survey was approved and administered by the municipal Health and Family Planning Commissions of the two sample cities. All collected data were only used to analyze aggregate results for this project, and were not permitted to be released to the public domain.
