Ethics approval and consent to participate
: This study received ethical approval under the Population Health Research Network’s Proof of Concept project, which included approval for developing and refining linkage methodology. Approval was obtained from Curtin University Human Research Ethics Committee (Reference: HR 15/2010) as well as approval from New South Wales Cancer Institute Human Research Ethics Committee (HREC/10/CIPHS/37) and Western Australian Department of Health Human Research Ethics Committee (HREC/2009/54). Ethics approval included a waiver of consent based on the criteria in the national statement on ethical conduct in human research.
