: This project is approved by the Geisinger IRB (2017–0238), and the Sutter Health IRB (for Palo Alto Medical Foundation Research Institute) (2017.134EXP). The following sites have elected to cede IRB oversight to Geisinger IRB, which has been accepted and approved: KPNW, KPCO, CHI, HealthPartners, Meyers Primary Care Institute, and Harvard Pilgrim Health Care.Patients and organizational stakeholders will be provided a study information sheet upon invitation to participate. Consent to participate in the interview will be obtained verbally after interviewer reviews the elements of consent and prior to asking any interview questions.
