: Ethical approval was obtained from both the Cabrini Human Research Ethics Committee (EC00239) and the Monash University Human Research Ethics Committee (EC00234). Consent to participate (for respondents who did not consent for their hospital records to be reviewed, and for non-respondents) was deemed unnecessary by the ethics department (Cabrini Human Research Ethics Committee). Cabrini Health approved access to de-identified patient information/hospital records without their consent as this was not deemed to infringe on patient confidentiality.
