Ethics approval and consent to participate
: The first page of the online survey contained a participant information sheet that explained the study and invited voluntary participation. Potential participants were given the option to consent to complete the questionnaire and by clicking the consent button they were taken to the questionnaire - or not consent, whereupon the questionnaire closed and they were thanked for their time. All questionnaires were anonymous, with no personally identifying information collected. The study received ethics approval from the Hunter New England Human Research Ethics Committee. In addition, site-specific approval was sought and granted from all participating LHDs.Ethical approval was granted by the Hunter New England Research Ethics Committee.HNE14/0716/4.03;NSW:HREC/14/HNE/124.
