: The authors asserted that all procedures contributing to this work complied with the ethical standards of the relevant national and institutional committees on human experimentation and with the Helsinki Declaration of 1975, as revised in 2008. And the study protocol was approved by the Ethics Committee of School of Medicine, Wuhan University (Number: 2016YF0096). All participants signed a consent to participate after reading the purpose, data using and personal information confidentiality rules of the study which were written on the front page of the questionnaire. The parents or guardians of participants aged 15–18 were also asked to sign the consent to participate. The statement of consent to participate was as following: I have read and understood the content and purpose of this questionnaire and agree to participate in this study.
