Ethics approval and consent to participate

: The study was submitted to the Medical Ethics Committee of the Radboud University Nijmegen Medical Centre under the file number (2016:2972). In the light of the Dutch Medical Research Involving Human Subjects Act, the Ethics Committee decided that extensive formal approval was not needed for this study. All participants were given both written and verbal information about the study, including the purpose and procedures, confidentiality of individual interviews or focus groups, the voluntary nature of participation and the opportunity to withdraw at any time. Interested clients and professionals participated after completing a consent form.
