Ethics approval and consent to participate
: The protocol was approved on 23/10/15 by NHS Blood and Transplant Research, Innovation and Technology Advisory Group. This approval included agreement to share anonymised NHS Blood and Transplant data. The study was approved by the Wales Research Ethics Committee 5 NHS research ethics committee (Reference 15/WA/0414 on 25/11/2015) and the NHS Blood and Transplant Research and Development Committee (NHS Blood and Transplant reference: AP-15-02 on 24/11/2015). Participants were provided with an information booklet and time to consider their participation. Written informed consent was obtained; participants were assured of anonymity and that they could disengage at any stage.
