Ethics approval and consent to participate
: All facility respondents were asked to consent to participate in the study. Prior to being surveyed, health providers were asked to sign consent forms that included details on the purpose of the study, potential benefits and potential risk, and clarified that the information provided would not be identifiable. Facility administrators who were asked questions about the services offered at the facility through a facility audit were asked for verbal consent to participate. All study tools including the consent procedures were approved by the Institutional Review Board at the University of North Carolina at Chapel Hill and by the National Health Research Ethics Committee of Nigeria (NHREC) in Nigeria.
