: The study was approved by Centers for Disease Control and Prevention institutional review board and appropriate Dartmouth-Hitchcock committees approved the study protocol and methods. Patient-level clinical data used to assess program effectiveness included no personal identifying information, therefore, consent was not required. Participants in case study interviews provided written signed consent. Interview data do not include personal identifying information. Darmouth-Hitchcock provided de-identified patient-level clinical data and de-identified patient navigation service delivery data to Centers for Disease Control and Prevention for analysis.
