Ethics approval and consent to participate
: The study was submitted to and approved by the hospital (Bnai Zion, Haifa, Israel) Institutional Review Board committee as exempt. This Institutional Review Board serves as the ethics committee for the hospital and approved the procedure for verbal consent. They felt verbal consent was sufficient because we guaranteed that participation was completely voluntary, unidentified, and confidential. We did not collect any information about the responders identification. Moreover, we guaranteed that we will not refer to any individual response but only to averages and standard deviation at the team level. Thus, the committee felt that the need for a written consent was not necessary in this case. Because the study was exempt, responses are anonymous and verbal consent is not documented or linked to respondents in any way. An informed consent letter was on the first page of each questionnaire, in which we explained that participation is voluntary and we would not refer to any individual responses, and that they could stop participation at any time.
