Ethics approval and consent to participate
: Ethical approval for this study was obtained from Maseno University Ethics Review Committee (Reference Number: MSU/DRPC/MUERC/00130/15) in Kenya. Informed consent was obtained in writing from all individual participants included in the study.
: In getting consent from participants, we made it clear to them that we were collecting information from records and their comments on the records, and that all of this information was going to be published so as to inform policy makers not only in Makueni County, but other counties as well. The information was also to be shared with the research communities.
