: Since this study utilized routinely collected aggregate programmatic data with no patient- or facility-level identifiers, no ethical clearance was needed. The purpose of such CQI initiatives is to improve health services and not research. This continuous quality improvement approach is a highly participatory approach that was undertaken with the Ministries of Health and hospital CQI teams taking the lead. The results have been summarized and communicated with relevant authorities to improve local care delivery.
