: Ethics approval was provided by the South African Medical Research Council (SAMRC) Human Research Ethics Committee before commencement of the study [protocol ID: EC007–5/2018, approval granted June 2018]. The study was carried out in accordance with relevant guidelines and regulations.Individual web-based information sheets and consent forms were provided at the start of the survey. Participants were only able to complete the survey if they were eligible to take part (determined by a qualifying question) and if the informed consent form was electronically accepted.Focus group participants were provided with an information sheet and consent form which explained the purpose, duration and conduct of the study before the focus group session. The focus group session commenced after participants had signed the informed consent form.
