: This study was conducted under the large cohort study in Anuradhapura District, Sri Lanka, Generating Evidence for Ending Preventable Maternal Deaths: the Rajarata Pregnancy Cohort (RaPCo). Ethical clearance to conduct the RaPCo study was obtained from the Ethics Review Committee of the Faculty of Medicine and Allied Sciences, Rajarata University of Sri Lanka (Reference no: ERC/2019/07).
